Company Overview
At Brookes Homecare Services, we understand that every person has a different story, different needs and different aspirations. That is why we do not believe in a one-size-fits-all approach to care.
Our purpose is to provide high-quality, person-centred care that enables vulnerable people to remain connected to their communities and continue living fulfilling lives in the environment they know and love. We support people of different ages with a range of care needs, while recognising their right to dignity, choice, independence and respect.
Our approach begins with listening. Before care starts, a qualified assessor works with the individual and, where appropriate, their family or friends to understand their needs, preferences, routines and desired outcomes. This information is translated into a clear and practical care plan that guides the support provided.
We aim to ease disruptive challenges faced by many clients of the health sector, by keeping people at home for as long as preferred by the family or the individual clients or couples.

Client Needs Assessments
The assessment is carried out by a qualified person who will put in place safety measures, to ensure the welfare and wellbeing of the client. The needs assessment is therefore client focused, and if necessary, may involve family and friends, to identify broader care needs and support, formulated into clear activities care plan that meet identified needs.
